- UnitedHealth Group: Founded in 1977 in Minnesota, it is the largest global health insurer, operating through UnitedHealthcare and Optum divisions. It initially focused on physician practice management before diversifying its offerings.
- Centene Corporation: Established in 1984 in Missouri, it serves underserved communities and offers Medicaid, Medicare, and marketplace health insurance plans.

Kaiser Permanente
